Home health care is a set of medical services and supportive care provided in your home by licensed healthcare professionals. At Hoosier Home Health we work closely with your physician to ensure that our services exactly match ‘just what the doctor ordered’.

We at Hoosier Home Health understand that having a say in planning your care is important to you. This is why we take a step-by-step approach to our intake and assessment process to ensure optimum care and no confusion for our clients and their family. Here is an overview of the process.

How Can We Help?
How The Hoosier Home Health Process Works
1. You or a family member requests Hoosier Home Health services. Hoosier Home Health will send your request to your physician for approval.
2. Within 48 hours after receiving a referral or orders from your physician, a Hoosier Home Health RN will call to schedule a
home visit for intake assessment.
3. If you agree to our services, the admission process begins and a comprehensive assessment will be completed.
4. Hoosier Home Health, along with your input, will develop a plan of care that meets your needs.
5. As a patient of Hoosier Home Health, we will coordinate all of your home care needs with you, your physician and other 
service providers.

Skilled Nursing
Our Registered Nurses (RN) are highly skilled and dedicated to providing quality, compassionate home care. An individualized, patient-centered plan of care is developed after the RN’s initial evaluation. In close collaboration with your physician, the RN's and LPN's provide skilled nursing services which may include.:
Assessment and monitoring of condition Pain management
Wound care Chronic disease management
Dressing changes Self care
Medication administration and instruction New diagnosis education
Patient and family education Restorative care
Bowel and bladder management Infectious disease management
Catheter insertion and care Post surgical care
Ostomy care    
Home Health Aide
Our Home Health Aides operate under the supervision of a Registered Nurse (RN). They are compassionate, dedicated individuals who are uniquely trained by Hoosier Home Health to provide the following services which may include:
Assistance with activities of daily living Personal care services such as bathing, dressing and grooming
Simple dressing changes Assistance with prescribed exercise programs
Light housekeeping    
Meal preparation
