As a RN Clinical Manager, you will:
• Lead and oversee all aspects of patient care and staff management, including assignments, patient assessments, referrals, and care plan updates.
• Coordinate home health services and ensure care begins promptly within regulatory guidelines.
• Manage physician orders, staff scheduling, and communication among care team members, patients, families, and physicians.
• Review and approve individualized care plans for clinical appropriateness, incorporating input from all providers.
• Conduct OASIS assessments, and regular OASIS reviews and guidance.
• Guide, coach and train direct care and clinical staff, and help interview and hire new team members to expand the agency.
• Collaborate with the Administrator on key performance metrics and contribute to quality improvement initiatives.
• Maintain relationships with referral sources.
• Engage in professional development activities.
• Perform other duties/projects as assigned

Required Experience/Skills:
• Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment.
• Home health experience is required.
• Management and people leadership experience, required.
• Current and unrestricted Registered Nurse licensure in the state of Indiana
• Current CPR certification
• Strong organizational and communication skills
• OASIS experience, required. OASIS certification (HCS-O, COQS, and/or COS-C), preferred.
• CMS PDGM billing knowledge and/or experience, preferred.
• Ability to successfully pass a national and state criminal history check, as required by IC 16-27-2-5*.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time salaried employment at the time of posting. The pay range may be higher or lower based on demonstrated job-related skills, knowledge, experience, education, certifications, etc., and may also be adjusted if Medicare provider status is achieved.
$72,500 - $89,900 per year
